Application for Replacement Facilities Access Card (FAC):

Alumni & Family / Community / Tenant / Associate / HKU SPACE / Senior Citizen / Foundation Member

Please read the terms below carefully before you fill in and submit this online form:

  1. Replacement card is subject to a fee of $150@ (non-refundable) and it has to be settled at the time of card collection by Octopus card or credit card.
  2. Replacement card will only be issued to member who still holds a valid membership.
  3. If you lost card(s) is / are found after the issuance of a replacement card, please return it / them to CSE as soon as possible.
  4. An email will be sent to you upon completion of our processing and you can then come to the reception counter of the Stanley Ho Sports Centre (10 Sha Wan Drive, Sandy Bay) to collect the card(s). Normally, the processing will take not more than 7 working days.
